HEALTH COLLEGE CONNECTION/SUMMER ENRICHMENT SELECTION CRITERIA
Rising Seniors (2013) and Graduating Seniors (2012)
ALL INTERESTED APPLICANTS MUST MEET THE FOLLOWING CRITERIA TO QUALIFY:

1. Be a full-time student in good academic standing.
2. Have an acceptance letter from a university, college or junior college. (Graduating Seniors only)
3. Had an outstanding disciplinary record during the (2011-2012) school year.
4. Be interested in a health care career.
5. Have a Grade Point Average of 3.0 or better.
6. Provide a copy of your official transcript.
7. Provide two references:
   A. One academic (teacher or counselor)
   B. One personal other than a relative
8. Identify in 150 words minimum, your career goal in essay form (Typewritten - 12 size font - Times New Roman)
